New Student Checklist

Welcome to Westfield State University. Now that you are an accepted student, here are a few things you need to take care of in order to have a smooth transition to the University.

DUE DATE:  as soon as possible -  click this link for account activation 

myWestfield is a secure site providing students with a one-stop shop for checking email, class registration, calendar, grades, housing registration and much more!

DUE DATE:  Optional but highly recommended!

EMERGENCY NOTIFICATION  REGISTRATION – Complete online on myWestfield

DISABILITIES NOTIFICATION FORM  (if applicable)
Complete online to find out about support services and reasonable accommodations for students with disabilities. 
DUE DATE: June 1 or soon thereafter.    Housing Accommodations - by May 12.

HEALTH/PHYSICAL and IMMUNIZATION FORMS – REQUIRED - Complete and return to Health Services
DUE DATE:   June 1   (IMPORTANT)

         **  Massachusetts law requires that all college students have certain immunizations before coming to school.
 
 

Federal Student Loan Borrowers must complete two online requirements at www.studentloans.gov - Entrance Counseling and the Loan Agreement for a Subsidized/Unsubsidized (MPN) Master Promissory Note.
DUE DATE:  June 21

       ** The federal student loan will not credit the fall semester bill until both items are complete.**

WSU360 – Complete your profile and explore your success network
DUE DATE:  July 1 or soon thereafter

FINAL HIGH SCHOOL TRANSCRIPT – Send to the Admissions Office  (First Year Only)
DUE DATE:   July 15

For Transfer students – Final College Transcripts are due no later than August 25, 2016

AP Scores – Send to the Admissions Office   (First Year Only)
DUE DATE:   July 15

         ** The university must receive official copies of your score report(s). You may have already requested this when you took the test. If not, please go the College Board website and request an official report be sent to Westfield State University. **

Dual Enrollment – Send transcript to the Admissions Office   (First Year Only)
DUE DATE:   July 15

HEALTH INSURANCE WAIVER OR ENROLLMENT FORM  - Must be completed online at www.universityhealthplans.com.  This site will allow the student to review our insurance brochure, enroll in our plan or waive our health insurance.
DUE DATE:  July 27

       ** Massachusetts law requires full-time college students to have primary health insurance. If you don’t already have insurance, or if the plan you have won’t meet your needs while at school, you can take advantage of the university-sponsored plan.
 

BILLING – Payment for the fall 2018 semester is due July 27
DUE DATE:  July 27, 2018

Student-Athlete Medical Requirements - Visit the Athletics page for details on Student health Form, Risk and Injury Waiver, Pre-Participation Medical History, etc.

All student-athletes must complete and submit the requirements linked and return to the athletic training room by the following deadlines:

August

FINALIZED FALL FIRST YEAR SCHEDULES WILL BE AVAILABLE ON AUGUST 20TH.    on MyWestfield 

WSU partners with BankMobile to deliver all student account refunds.  All students are required to set up their Refund Preference Selection through the BankMobile website.

Sexual Assault Prevention for Undergraduates – REQUIRED - Complete through MyWestfield

         ** Understanding Sexual Assault is an interactive online platform designed to educate about the elements of healthy relationships, the importance of sexual consent, and the role of bystanders in creating safe and healthy communities.
