The Proof of Residency Form will be sent to you after submitting the required Enrollment Deposit. The Proof of Residency Form is required of all new students to Westfield State University and must submitted by June 1.

Federal Student Loan Borrowers must complete two online requirements at www.studentloans.gov - Entrance Counseling and the Loan Agreement for a Subsidized/Unsubsidized (MPN) Master Promissory Note.

Health Insurance Waiver or Enrollment Form  - Must be completed online at www.universityhealthplans.com.  This site will allow the student to review our insurance brochure, enroll in our plan or waive our health insurance.
