Employee Change of Information
Changes of information to your employment file with Human Resources can be completed using your online Employee Self Service (ESS) account or submitting via forms.
Changes via Employee Self Service (ESS)
Changes you can make as an active employee using HRCMS Employee Self Service (ESS):
- Personal information: Address, contact info, and ethnicity
- Direct Deposit
- Federal taxes (not State)
- Talent Profile: Education and qualifications
- COVID-19 Vaccination
- Consent to receive your annual W2 and 1095-C electronically
Changes using Change of Information form
Changes you can make using the Change of Information form:
- Name changes
- Marital status
- Military service
- Gender
- Highest education level
Completed forms should be submitted to Payroll in-person, via interdepartmental mail, or email payroll@westfield.ma.edu.
Update your ethnicity
You can update your ethnicity online using your HRCMS Employee Self Service (ESS) account or you may opt to report the change using the Ethnic Group Change form.
